Bachelor's Degree, official transcript of highest degree, two letters of recommendation using the SHRP-IDS recommendation form, personal statement, copy of professional license/certificate (if applicable), resume/curriculum vitae, and minimum 3.0 GPA. Applicants with no-U.S. degree must have transcript evaluation.
